New Rite Aid program creates health care team

BUFFALO, N.Y. – In an initiative called a first for the drug store industry, Rite Aid Corp. has introduced the Rite Aid Health Alliance, a new program in which various health care professionals work as a care team to help people with chronic and polychronic health conditions meet health improvement goals set by their doctors.
Rite Aid on Monday said it has launched the Rite Aid Health Alliance in several markets — in Buffalo, N.Y.; Greensboro and High Point, N.C.; and the Los Angeles area — following a pilot in which the program delivered more than 2,300 coaching visits to almost 1,500 enrolled patients.
The integrated care model leverages the combined expertise of community pharmacists along with specially trained in-store health care coaches.
Patients with chronic conditions and polychronic conditions such as congestive heart failure, chronic obstructive pulmonary disease (COPD), high cholesterol and diabetes are recommended to the program by their primary care doctor. Rite Aid pharmacists and the care coaches, located in Rite Aid pharmacies, work with the physician and patient on an ongoing basis to improve the patient's overall health and self-management abilities. Care team members collaborate with the patient to establish health goals, eliminate barriers and create a personalized health care action plan in coordination with the patient's physician.
John Standley, Rite Aid's chairman and chief executive officer, announced the Rite Aid Health Alliance along with president and chief operating officer Ken Martindale and executive vice president of pharmacy Robert Thompson at a Rite Aid store in Buffalo. Also at the event was Dr. Raul Vazquez, president of Greater Buffalo United Accountable Healthcare Network and one of the alliance's first partners.
"We're committed to actively working with our customers to keep them well, and that includes developing new ways to improve access to high-quality health care," Standley stated. "Through our innovative Rite Aid Health Alliance program, we will help patients with the resources and support necessary to successfully manage their conditions and improve their overall health and well-being."
Besides the Greater Buffalo United Accountable Healthcare Network, Rite Aid has formed partnerships for the alliance with High Point, N.C.-based Cornerstone Health Care and Glendale, Calif.-based Apollo Medical Holdings Inc. to provide these services in select Rite Aid pharmacies in Greensboro and High Point, N.C., and in the greater Los Angeles area, respectively.
In those areas, Rite Aid provides the care coaches on the team through a partnership with Health Dialog, a provider of health care patient support, analytics and decision support. Health Dialog coaches, available in Rite Aid pharmacies, specialize in behavior change to help patients address lifestyle health issues.
