Cloud-based PrescribeCare is an electronic pharmacy “care plan” that pharmacists can use to digitally communicate with other care providers, reducing redundant data entry, improving care coordination, and earning community pharmacies compensation for their care interventions.
The practice management system-agnostic solution interfaces with more than 50 pharmacy management systems to analyze dispensing data in real time, giving the pharmacy team insight into the needs of their patients and streamlining medication reconciliation, according to PrescribeWellness.
“Our pharmacy profession is rapidly expanding the breadth of services we deliver to our patients by providing enhanced clinical interventions, with demonstrable positive outcomes and ROI,” stated Peter Kounelis, vice president of the Elevate Provider Network at AmerisourceBergen. “PrescribeCare is a critical component in supporting this broader service model by facilitating the capture and sharing of the necessary components, such as patient encounter recording, patient education and information sharing with other health care team members.”
Built using the Pharmacist eCarePlan, a nationally recognized standard, PrescribeCare allows pharmacies to more quickly exchange information with other members of the care team. Through the solution, pharmacists can prioritize patient concerns or goals, add them into the patient record, and share medication-related information. The plan can be printed for the patient to share with primary care doctors as well as caregivers.
PrescribeWellness noted that community pharmacists also can use PrescribeCare to identify interventions and services a patient needs. In turn, pharmacies can drive patient solutions such as medication synchronization and reconciliation, adherence monitoring and coaching, compliance packaging, immunizations, Medicare plan reviews, prescription delivery, communication to the electronic health record to other health providers and billing for enhanced services.
“PrescribeCare further advances community pharmacies’ ability to identify and manage patient care encounters daily, collaborate more easily with physicians and health systems, and uncover nondispensing revenue opportunities,” commented Al Babbington, chief executive officer of PrescribeWellness. “We’re excited to be empowering pharmacies in the Elevate Provider Network to go ‘beyond the fill’ with more enhanced service offerings.”
Launched at AmerisourceBergen’s ThoughtSpot 2015 conference, the Elevate Provider Network is designed to help independent pharmacies improve profitability via enhanced patient care, better business practices and centralized data to support reimbursement discussions with payers.
