OutcomesMTM rolls out Connect Platform

DES MOINES, Iowa – Medication therapy management service provider OutcomesMTM has launched its Connect Platform, a web-based system that will serve as the hub for all documentation, billing, reporting and administrative functions for its MTM programs.

The company said Monday that more than 50 health plans have adopted the OutcomesMTM unified platform, which connects such plans with over 70,000 chain, independent, consultant and health system pharmacy providers nationwide to streamline and expedite MTM service delivery and administration.

"Part of our ongoing commitment to the face-to-face difference in MTM is continually developing ways to help our network pharmacists deliver exceptional services in a more efficient manner," stated Jessica Frank, PharmD, vice president of operations at OutcomesMTM. "Connect represents 14 years of feedback and refinement, the sum total of our expertise in delivering successful MTM programs since 1999. It is the most advanced system of its kind.”

In OutcomesMTM programs, participating pharmacists receive alerts and information on medication use patterns, plus guidance on working with patients and doctors to close gaps in therapy. The Connect Platform serves as the software behind these consultations.

"The patient-centric structure within the new platform will drive efficiencies at network pharmacies and closely align the goals of our health plan clients with local pharmacists in our program," explained Cynthia Kesteloot, OutcomesMTM's vice president of technology. "And even though the platform embodies hundreds of improvements big and small, the framework and processes for MTM services will still be familiar to pharmacists who have previously participated in OutcomesMTM programs."
